Here are various forms and their descriptions:

• Emergency Medical Form (PDF) - Basic Medical Information for the Player.  This form is capable of being electronically filled out on the screen and then printed out if you wish.

• Player Contract (PDF) - Contract needed to be filled out to play in the league. This form is capable of being electronically filled out on the screen and then printed out if you wish.

• Parental Code of Conduct (PDF) - States the expectations for parents at all Lefty Grove Baseball, Inc. events.  Parents are to serve as models for the youth and adhere to the code of conduct.

• Coaches' Code of Conduct (PDF) - States the expectations for coaches at all Lefty Grove Baseball, Inc. events.  Coaches are to serve as models for the youth and adhere to the code of conduct.
